Dear [NAME],

I am writing to discuss my recent request for disability accommodation in your class, which unfortunately, I have not received or have been denied during a particular assignment. As a person dedicated to driving my own success and self-advocacy, I am aware that facilitating some accommodations is an additional responsibility that requires collaboration. However, I also believe in academic integrity, equity, and our fundamental rights as students to emphasize the importance of adhering to legal obligations that guarantee our equal access to education.

Having said that, under the Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act, educational institutions in the United States are mandated to provide reasonable accommodations for students with documented disabilities. I have submitted paperwork and have worked with the Office of Disability services to ensure my paperwork is accurate and up to date. These accommodations are designed to create an inclusive environment for me that allows me to participate fully and demonstrate my knowledge and abilities without unnecessary barriers that my disability creates for me.

I understand that misunderstandings can occur, and I remain committed to collaborative problem solving under the assumption that the University will stand by its commitment to me also.

At this moment, I feel that I need to address this matter more formally with the University, and I am writing to inform you that I intend to initiate an official student grievance process with the Office of Disability Services. I believe that through open communication and collaboration, we can resolve this issue amicably and ensure an equitable academic experience.

If I find that the formal grievance process does not resolve my issue, I will be compelled to file a complaint with the Office for Civil Rights (OCR) to investigate the violation of my rights as a student here.

My goal in writing this letter is to promote an understanding of disability accommodations not just for myself, but for future students also. I remain optimistic about our potential collaboration to address this matter in a way that aligns with the University’s core values. Please inform me of your availability for a discussion to address any concerns or provide further clarification on my needs. Thank you for your time and consideration.

Sincerely,
